FODMAPs are certain short-chain carbohydrates that may be poorly absorbed in the small intestine. They can draw water into the bowel, and gut bacteria ferment them to produce gas. Water and gas can stretch the bowel.
These are normal processes in people with and without IBS. The amounts vary. Some people with IBS experience visceral hypersensitivity: ordinary stretching can feel uncomfortable or painful. Food, gut sensitivity and other factors can all contribute to bloating and symptoms.
Choosing a lower-FODMAP portion can reduce the carbohydrates available to draw in water and ferment. The aim is to reduce FODMAP exposure, rather than simply eat less food. This concerns water in the bowel, rather than general fluid retention.

Gut-directed hypnotherapy has also been studied for IBS. Two different trials help explain the evidence.

In a trial of 74 adults with IBS, 72% receiving gut-directed hypnotherapy, 71% following a low FODMAP diet and 72% receiving both achieved the study’s improvement threshold after six weeks.
The threshold was at least a 20 mm improvement in overall gastrointestinal symptoms on a visual analogue scale. The groups did not differ significantly on the primary symptom-change comparison. This was a clinician-delivered intervention, not Nerva, and the trial did not establish that the treatments are equivalent.

In a separate trial, 81% in the Nerva group versus 63% in an active-control group achieved at least a 50-point decrease on the IBS Symptom Severity Scale at the end of the program.
The study randomized 244 adults with IBS; 240 received a 42-session digital program with or without gut-directed hypnotherapy. The comparator was not a low FODMAP diet. These percentages use a different outcome measure from the 2016 trial. The response rates cannot be directly compared across these trials.
