How to refer a patient to Nerva

Clinicians can use Nerva’s online referral form to connect a patient with the program. The steps below explain the process; the video shows how to introduce the program and discuss expectations with your patient.

Refer a patient in four steps

1. Discuss the program and obtain consent. Explain the referral and obtain your patient’s consent to share the information requested by the form. The form asks you to confirm that consent and the required notices have been provided. Give the patient an opportunity to ask questions before you submit their details.

2. Complete the secure referral form. Open the Nerva referral form and enter the required patient and referring-clinician details. Use this secure form for patient information. Follow the current fields and instructions shown on the form, and check the details before submitting.

3. Nerva supports onboarding. Nerva handles onboarding, follow-up and program support. For current access options, eligibility and costs, see the clinician information page. Discuss the available route with your patient rather than assuming that a referral establishes insurance coverage or confirms eligibility.

4. Review progress the patient chooses to share. Patients can opt to share self-reported IBS symptom and wellbeing check-ins with their clinician through Nerva’s reporting system. Explain that this sharing is optional. These check-ins can support a conversation about progress; they do not replace your clinical assessment.

Is this an EHR referral?

These instructions describe Nerva’s web referral form. Do not assume that completing the web form also creates an order or record in your organization’s EHR.

Before you share the referral

Confirm that your patient understands what you are referring them to, why you think it may be appropriate, and how to find current program information. Separate the decision to refer from any promise about cost, coverage, access timing or symptom improvement.
